Be it enacted by the Sangguniang Panlalawigan in session duly assembled that:
SECTION 1. TITLE This Ordinance shall be known as "AN ORDINANCE PROVIDING FOR THE POLICIES AND FEES OF THE CAROTID DUPLEX SCAN PROCEDURE AT THE PANGASINAN PROVINCIAL HOSPITAL, THEREBY AMENDING PROVINCIAL TAX ORDINANCE NO. 1-2012 BY ADDING PAR. 1.2c AND 1.2d UNDER SECTION 46 (a) (1.2) THEREOF";
SECTION 2. The amendment shall be as follows:
SECTION 46. Health Service Fees
a. Fees/Rates 1.2 Use of Equipment 1.2c Policies in the Use of the Carotid Duplex Scan shall be as follows:
a. Carotid Duplex Scan will be performed at the Heart Station of the Pangasinan Provincial Hospital by a vascular medicine consultant who is a Board Certified Cardiologist with one year training in Peripheral Vascular Medicine until a trained vascular technician as required by the Philippine Society of Vascular Medicine is available.
b. All Carotid Duplex Scan studies are on cash basis. The laboratory request will be accomplished by the nurse- on-duty and will be paid at the billing section prior to the procedure. The official receipt will be given to the Heart Station technician for scheduling.
c. Schedule for the performance of Carotid Duplex Scan will be every Tuesday at 3:00 to 5:00 p.m. and Friday at 1:00 to 3:00 p.m.
d. There would be no reader's fee for charity and indigent patients.
