The title page footer should use Template:title_page, like this:

Parameter notes:

• Do not delete a parameter - leave the field blank if there is nothing to enter.
• author - link to the author page on wikisource.
• provenance - link to the location of the site where it was taken from or note how you got it.
• loc - Look at the Category:LC- page for the proper category.
• wikiref - provide a link to the author and work separately, if they are available.
